Forms and Lists
The following are the forms and lists that will be needed for attending Camp Bil-O-Wood:
1. Registration Form – MUST be completed by all campers and junior counselors for registration in each season. The form MUST be signed by a parent or guardian. Please send to the address listed on the form.

2. Medical Form – MUST be completed for every camper each season. MUST included the families insurance information and be signed by the family doctor. Please send to the address listed on the form or bring it to camp at the beginning of your camper’s session.

3. Junior Counselor, Assistant Counselor and Counselor – Should be completed by all those who apply and sent to the address listed on the form.
